The undersigned acting on behalf of the applicant hereby declares that all information provided above is true and shall undertake to notify Flexxo Trading of any changes. In consideration of the extension of credit by Flexxo Trading to the above applicant, the understand and agrees that all invoices are due net 30 days and are subject to 1.5% per month (18% per annum) finance charge after 30 days. In the event of any discrepancies in Delivery Orders, Invoices and Account Statements, Flexxo Trading must be notified within 7 days of receipt. Flexxo Trading shall at its sole discretion reserves the right to approve and retract all credit facilities without prior notice. I/We authorize consent and/or undertake to procure the authorization/consent of its relevant employees and/or agents for personal information including sensitive personal data, being processed by and where required, disclosed to classes of third parties as identified by Flexxo for the purposes of Flexxo providing the said products and services.

By this consent, I/We understand and agree that:

i)You may conduct credit/ trade check, CCRIS and DCHEQUE checks on us and where applicable with CRA at any time for as long as I/we have a trade relationship with you or where any dues remain unpaid and outstanding with you, for anyone or more of the following purpose:

*Opening of account *Credit/ Account monitoring

*Debt recovery *credit/Account evaluation *Credit/Account review

*Legal documentation consequent to a contract or facility granted by you.
